Update Request: Requesting Most Recent Year of Limited Data Set (LDS) Files under Existing DUA

LDS Requesters must submit to CMS the following request materials:

Written request letter
LDS Order Form (under the "Downloads" section)
Check or money order for payment in full

LDS file request packets should be sent to:
