Items 1,3,4,5 come from the CWF Beneficiary
Master Record; item 2 comes from the FI/Carrier
claim record.  MSC is assigned as follows:

to distinguish this CWF-derived field from the
EDB-derived MSC (BENE_MDCR_STUS_CD).
